Leadership Team Hub:
The Leadership Team (LT) Hub displays your saved and submitted Chapter Success Meeting (CSM) forms. Your role determines which chapters you can access and what you can do with their forms.
Access levels
• Manage forms: Create, submit, view, and print forms. Edit or delete forms while they are in Draft/In Progress status.
• View and print only: View and print forms. Cannot create, edit, submit, or delete them.
Permissions by role

Once a form is submitted
Submitted forms are locked and cannot be edited or deleted. If a submitted form needs changes, please contact your Regional Office.
Creating a new form
If your role allows you to create forms, click the plus (+) icon in the top-right corner of the page to start a new CSM form.
