Visitor registration form not mobile-optimized

The visitor registration form does not work very well on mobile. Please put it above or below the chapter information. One issue is that the email opt-in pop up menu choices only appear at the location of the menu when the screen is fully zoomed out. When zoomed in, the Yes/No choices appear somewhere off to the left, which might be invisible to the user. This is especially problematic because the form is so difficult to read zoomed out due to the aforementioned placement issue.
