Should  be able to hit enter and drop down a line to enter address in traditional format. Also, some client don't share email or don't have one. It should not be required to move on with the referral.

  @kevin - thank you so much for the suggestion for the address fields - this was added to the list for future consideration!  As for the email, only the phone number is required - the email address should be optional  :- )
