Slips issues.
Should be able to hit enter and drop down a line to enter address in traditional format. Also, some clients don't share email or don't have one. It should not be required to move on with the referral.

@kevin - I just tested through this and:
1) I was able to successfully hit enter in the Address box (and comments) to go to the next line.
2) The PHONE NUMBER is mandatory, but the email address is not
Can you please confirm on your end? If not, can you please provide more details about the type of phone and operating system you are using and we can look into this further as a support issue.
